You can access SERS most commonly used forms by clicking the links below:
- Employee Beneficiary Change Form - Use this form if you are an active or vested employee and would like to update your beneficiary.
- Retiree Beneficiary Change Form - Use this form if you are a retiree and would like to update your beneficiary. If you have selected Option D or E, you are unable to change your beneficiary.
- Change of Address or Name Form - Use this form if you would like to change your address or change your name.
- W4P Tax Withholding Form - Use this form to update your Federal Tax withholding.
- Direct Deposit Form - Use this form to start or change your direct deposit.
- Withdrawal Form - Use this form if you are a terminated employee and would like to withdraw your contributions.
